Please use the following form if you need your counselor to release or work with someone outside the counseling office. This will allow your counselor to work with and share information as specified by you to any person(s) in the community as well as work with instructors and other CSI personnel as needed. 

Please speak to your counselor in further detail about this form and what your needs are to ensure that you will need this form as it is not required for your sessions and is a personal decision. Thank you
